  • Please fill out the online Biography form prior to your appointment
  • Sign the "Informed Consent and Policies" form in the online portal
  • Upload your insurance card or bring a copy to your first appointment
  • If visits will be through Telemedicine please see the Telemedicine section of the informed consent document for more information.​​
